This position coordinates patient admissions to the hospital, and all patient transfers between patient care areas. This position may be required to access and administer medications within their scope of practice and according to State Law.

Responsibilities

  • Performs thorough on-site or telephonic assessment of potential patients including chart review, clinical assessment and interaction with physicians, nurses and therapists directly involved in the patients care.
  • Proactively assists hospital discharge planners with early identification of patients who may qualify for and benefit from an acute inpatient rehabilitation facility.
  • Coordinates the various components of the pre-admission process inclusive of completion of the pre-admission assessment tool and appropriate review by the rehab physician with signature.
  • Educates patients, family members or significant other, referral sources, hospital personnel, external payers and internal/external case managers as to the rehabilitation program and the effective and efficient utilization of services.
  • Maintains a thorough knowledge of the rehabilitation unit's services and capabilities. Works as a liaison between the internal rehab team and external referral sources.
  • Responsible for developing census as defined through targeted goals of the marketing plan and developing referral relationships with emphasis on face to face contacts.

  • Demonstrated human relation and effective communication skills.
  • Ability to perform the essential functions of the job as outlined in the position description.
  • Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.
  • Must demonstrate knowledge of the principles of life span growth and development.
  • Ability to assess data regarding the patient's status and provide care as described in the department's policies and procedures manual.

Required

  • Four years of related experience, with two of those years in post acute services.

Licenses Required

  • Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
  • One of the following
    • Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.
    • Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
    • Current licensure to practice as an Occupational Therapist in the State of Utah.
    • Current license to practice as a Physical Therapist in the State of Utah, or obtain one within 90 days of hire under the Physical Therapy Compact if switching residency to State of Utah. Must maintain current Physical Therapy Compact (multi-state) license if residency is not being changed to Utah.
    • Current licensure to practice as a Speech Pathologist in the State of Utah.

  • This position involves intensive work that may exert up to 100 pounds and may consistently require lifting, carrying, pushing, pulling, or otherwise moving objects, such as medical equipment, or patients while providing medical care. This position may be exposed to infectious diseases and may be required to function around prisoners or behavioral health patients.
