This position serves patients in three rural counties: Emery, Grand, and Carbon, and requires the nurse to live in Price and report to the Price office. The role involves some travel to Salt Lake City's Huntsman Cancer Hospital for orientation, training, and monthly meetings. The nurse will work closely with the Nurse Practitioner in the Price office, completing home visits with the Advanced Practice Clinician (APC) to support research initiatives and provide supply coordination/delivery when needed. Responsibilities also include community outreach and resourcing, educating home care staff, tracking data to support research initiatives, and coordinating care between primary care, the oncology team, and community hospitals.
