Department Specific Overview:
• Collaborate with inpatient Nephrology and Case Management teams to secure dialysis setup and resumption of services within community centers
• Coordinate dialysis clinic placement for patients in the Emergency Department who require ongoing treatments
• Initiate and pursue authorization for outpatient plasma exchange during the patient’s inpatient stay
• Schedule outpatient plasma exchange appointments including treatments, labs, imaging, and catheter removal
• Maintain accurate and up-to-date Case Management documentation
• Clear and accurate communication with inpatient and outpatient care teams
• Coordination and interaction by utilizing portals and external systems
• Communicate coordination of care to patient/representative
• Coordinates with and educates external partners in care
• Supports operational alignment with organizational goals under the supervision of department leadership
• May perform other duties as assigned; participate in department quality improvement projects
