The Heal at Home program, part of University of Utah Health’s Population Health division, delivers post-acute care to patients in their homes, supporting recovery after surgery or hospitalization. Partnering with Community Nursing Services, the program spans over 13 surgical specialties and focuses on reducing hospital stays, improving outcomes, and enhancing patient satisfaction through coordinated, home-based care and virtual support.
