EO/AA   The HCI Huntsman at Home program is a new initiative at the leading edge of palliative healthcare delivery which is strengthened by being integrated with the acute Hospital. This is a new initiative by the HCI that will provide multidisciplinary acute clinical care, palliative care, and Hospice care support to HCI patient’s in the patient’s own home. The Huntsman at Home care will take place in the patient’s own home 7 days a week, 365 days a year supporting the goals of admission avoidance, reduction of emergency room vists and early discharge support from the hospital, in both palliative and end of life care.   The post holder will report operationally to the Manager of Huntsman at Home and have professional line management to the HCI APC Director and Huntsman at Home leads. They will work collaboratively with the Strategic Director of Huntsman at Home. They will have weekly check-ins with the Huntsman at Home APCs and Leads and work collaboratively and in consultation with the medical director. Nurse Practitioner (NP) role is a new position and as such will entail spending time working with patients, the local home health agencies, hospitals and other community services. The role in this role will cover the 3 rural Utah counties of Grand, Emery and Carbon. It would be an advantage to this role if the candidate lived in or close to these communities although this is not required. If not, the NP will need to travel to these counties on a weekly basis initially for at least the first 6 months of operations which may require overnight due to travel times. Travel is reimbursed. Once the role is established in the 3 counties frequency of travel may be reduced as telehealth visits are increased. The APC will demonstrate a high level of expertise within the palliative care and oncology fields in the assessment, developing a care plan and implementation of a care for the patients on service care. The APC will also be responsible for Advance Care Planning and Goals of Care conversations with HCI patients and their families and will oversee the patient care in all aspects of the program. They will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service, as well as policies, protocols, guidelines and service developments in collaboration with Huntsman at Home medical director and team leads. To ensure the safety and well-being of service users, and always work in a manner that promotes dignity and human rights through the adoption of person centered care principles.
